• $65 for follow up (up to 15 minutes in length)

• $125 for follow up (up to 30 minutes in length)

• $175 for follow up (up to 45 minutes in length)

  If you have an insurance provider that we are not in network with, we will provide you with Superbills to submit to your insurance provider for reimbursement.​

• Bariatric Psychiatric Clearance: $250  

• Disability Assessment Forms: $100  

• Family and Medical Leave Act (FMLA) Forms (Initial): $150  

• FMLA Follow-up/Update Form: $75  

• Jury Duty Accommodation Letter: $35  

• Insurance Forms: $35  

• Special Accommodations Letter: $35  

• Emotional Support Animal Letter: $35  

• Disabled/Handicap Form: $35  

• All requests are processed within 14 business days, subject to the psychiatric nurse practitioner's discretion. To expedite your request, ensure that all demographic information is provided in advance. Please note that we are not responsible for any denials and/or rejections. For documentation requests, clients must have been established and active for at least three months before making such requests with the exceptions of the Bariatric Clearance. 

• Legal matters requiring the testimony of a mental health professional may arise unexpectedly. Such testimony can sometimes negatively impact the therapeutic relationship between the patient and practitioner. Therefore, if a legal evaluation or testimony is needed, I require you to engage an independent forensic psychiatrist or psychologist. However, if I am deposed or subpoenaed to testify or appear in court on your behalf, you will be responsible for my court fees, which are a minimum of $1,080 for a half day or $2,160 for a full day, payable directly to IAW Behavioral Health & Wellness.
